Membership is for 1 year beginning on the date of first payment. A 3% charge is added to all accounts that draft from a credit or debit card. Membership will automatically renew on the anniversary date unless a written request to cancel is received 30 days prior to the renewal. Annual membership fee is subject to change upon written notice. Membership must be current to receive all discounts. Whitening discount does not apply to Kor whitening. It is the member’s responsibility to schedule and keep all appointments offered as part of the Dental Membership Program. Missed appointments and appointments cancelled with less than a 24-hour notice will be subject to the office missed appointment fee. All periodontal maintenance appointments will be scheduled in 3 month intervals.
